Congratulations! Please consider this letter your notice of acceptance to the Dubuque 2008 Blowout Soccer Tournament to be held July 11th, 12th, & 13th at the Dubuque Soccer Complex located on the corner of JFK Road and NW Arterial in Dubuque, Iowa. The entire Tournament Committee, as well as the Dubuque Soccer Club, is pleased you have selected to attend our tournament! Schedules and Brackets will be posted on-line at www.dubuquesoccer.org for you to refer to and print off today and attached in this email.

Please send the following to the Tournament Director by July 1st:

____ Copy of State approved player/coach roster; (Bring updated roster to the Tournament; Registration will keep both copies). Please list any guest players and their information below rostered players. This is not a final roster until you turn it in the day of your first game.

Please fax information to the Tournament Director, Jason Berna at (563) 589-3425.

Please bring the following with you to the Tournament:

____ Updated State Approved Roster. List guest players and their information below rostered players List 2 contact names and phone numbers that can be reached during the tournament for each team.

____ Travel papers for out of state teams.

____ Signed State or Club approved Medical Release Forms for each player. Must be signed by the parent/guardian.

____ Player pass cards in Alphabetical order with coaches' cards at the end. Remember…NO CARD, NO PLAY!! All coaches must have a state approved card to be on the sideline.
